Please do not submit Social Security numbers, driver license numbers, full dates of birth, immigration documents, medical records, account passwords, or bank information through this website.

Date of birth and your Washington driver number (WDL) are not collected on this website. Staff will ask for them in person only if they are needed for the approved enrollment process. If you do not have or cannot find your WDL number, staff can help with the next steps.

Word of Life collects information to arrange program services and keep required program records. Information may be shared with authorized service providers as needed for those purposes. Ask staff for the full privacy notice or help with this form.
